The UCSF Clinical Laboratories have limited resources to provide specialized report requests. Additionally, there are some limitations: results are purged after 3 years, so this data is no longer available in the laboratory information system. Results are archived after a year, which increases the difficulty in running reports that include data over a year old: this requires running a separate report to capture this data. STOR does not purge data, and has the ability to run reports - please refer to the STOR/CDS home page, and click on the "Forms" link at the top of the web page.

Due to UCSF Policy, the Clinical Labs cannot respond to requests for data for research purposes. Please submit the request through HIMS - please refer to the STOR/CDS home page, and click on the "Forms" link at the top of the web page.

The Report Request form is available below. A fee of $100/hr with a minimum of 2 hours will be charged for reports. The filled out request form should be sent electronically to Enrique.Terrazas@ucsf.edu or hard copy to Enrique Terrazas at UCSF Box 0451.
